Abstract

The Community Personal Needs Assessment (NEEDS) Questionnaire (Sapuppo et al., 2023) is a self-assessment instrument designed to identify unmet needs among young stroke survivors. This 175-item questionnaire was developed through a comprehensive literature review, incorporating insights from prior unmet needs assessments. It aims to cater to stroke survivors aged 18-55, at least two months post-stroke, and residing in the community. The questionnaire assesses seven domains: Healthcare Experience, Impairments from Stroke, Everyday Activities, Work or Study, Finances, Relationships, and Social Participation. Participants respond with yes/no to indicate an unmet need and then rate its significance on a six-point Likert scale.

Format

The NEEDS Questionnaire is administered electronically. It includes demographic questions followed by 175 items organized into seven domains:

  • Healthcare Experience: Questions 6 to 39

  • Impairments from Stroke: Questions 40 to 80

  • Everyday Activities: Questions 81 to 94

  • Work or Study: Questions 95 to 116

  • Finances: Questions 117 to 124

  • Relationships: Questions 125 to 137

  • Social Participation: Questions 138 to 146

For each item, participants answer yes/no to indicate the presence of an unmet need. If an unmet need is identified, they then rate its significance using a six-point Likert scale, where 1 represents “not significant” and 6 represents “highly significant.” In some instances, additional questions are posed to further explore the factors contributing to an unmet need.
