Case Management

Case Management is the function of long-term or short-term interaction with a client that is focused on monitoring and overseeing treatment programs and general welfare. This is a function that is often used in medicine, psychiatric care, child welfare agencies, eldercare agencies, and other types of social service functions. A case manager is responsible for maintaining personal contact with the client and their immediate family, providing assistance as needed and communicating with other involved caregivers, agencies, etc.
