De Nederlandse consensus Health Assessment Questionnaire Disability Index (HAQ-DI) is de Nederlandse vertaling van de Engelstalige Health Assessment Questionnaire (HAQ) van de Universiteit van Stanford. Tot consensus is besloten omdat er 3 soortgelijke vragenlijsten beschikbaar waren en dit voor verwarring zorgde. De HAQ-DI is een door de patiënt zelf in te vullen vragenlijst, die beperkingen bij het uitvoeren van activiteiten van het algemeen dagelijks leven (ADL) meet.
