Administration Method and Scoring
1. For items 1-4, 8-10: Select one option, with selection 1, 2, 3, and 4 scored as 1, 2, 3, and 4 points respectively.
2. Item 5 is scored based on the support from A, B, C, D, with scores from none (1 point) to full support (4 points).
3. For items 6 and 7, a response of “no source” scores 0 points, while selecting sources scores 1 point for each source chosen.
