De HOOS-PS is een vragenlijst voor het evalueren van symptomen en beperkingen bij patiënten met heupklachten. De vragenlijst bestaat uit 5 items en is de verkorte weergave van de HOOS en is samengesteld uit de subcategorie ADL-activiteiten en Sport & Recreatie. De verkorte versie gaat over de hoeveel moeite die het uitvoeren van een activiteit gedurende de afgelopen week gekost heeft. De scoring wordt gedaan aan de hand van een 5-punts Likertschaal (0-4), waarbij een hogere score meer moeite aangeeft.
