Purpose

The EPDS is used to identify symptoms of postpartum depression and help in the early detection and intervention of affected individuals.

Test Year

The original EPDS was developed in 1978.

Administration Method and Scoring

Respondents are asked to reflect on their feelings over the past week and select the answer that best reflects their experience. Each item is scored from 0 to 3, with items 1 and 2 being reverse-scored. The scoring for the items is as follows:
1. (1) Same as before. (2) Not as much as before. (3) Definitely less than before. (4) Not at all.
2. (1) Same as before. (2) Not as much as before. (3) Definitely less than before. (4) Not at all.
3. (1) Most of the time. (2) Sometimes. (3) Not often. (4) Not at all.
4. (1) Not at all. (2) Very rarely. (3) Sometimes. (4) Often.
5. (1) Quite a lot of the time. (2) Sometimes. (3) Not often. (4) Not at all.
6. (1) Most of the time I can’t cope. (2) Sometimes I can’t cope as well as usual. (3) Most of the time I can cope as well as usual. (4) I have been able to cope well all the time.
7. (1) Most of the time. (2) Sometimes. (3) Not often. (4) Not at all.
8. (1) Most of the time. (2) Quite a lot of the time. (3) Not often. (4) Not at all.
9. (1) Most of the time. (2) Sometimes. (3) Occasionally. (4) Not at all.
10. (1) Quite a lot of the time. (2) Sometimes. (3) Very rarely. (4) Not at all.

Reliability and Validity

The Chinese version of the EPDS, compiled by Dominic T.S. Lee et al. at the Chinese University of Hong Kong in 1998, demonstrates good sensitivity and specificity. A score of ≥10 is used as the diagnostic standard, yielding a sensitivity of 82%, a specificity of 86%, a positive predictive value of 44%, and a negative predictive value of 97%. To reduce false negatives, a score of ≥7 may be employed. The 9/10 cutoff is significantly better for diagnosing depression compared to the traditional 12/13 cutoff, which results in higher false negative rates.
