De WPAI brengt middels 6 vragen de afname in werkproductiviteit en activiteiten van de afgelopen 7 dagen in kaart. Er zijn 3 vormen van de WPAI, namelijk de WPAI-GH (General Health), waarbij het gaat om de algemene gezondheid van de medewerker, de WPAI-SHP (Specific Health Problems), waarbij rekening wordt gehouden met een specifieke aandoening/ziekte, de WPAI-GH/SHP (een combinatie van de -GH en -SHP) en de WPAI+CIQ:AS (Allergy Specific, inclusief CIQ (Classroom Impairment Questions)), waarbij rekening wordt gehouden met de bijkomstigheid van allergieën. Alle versies geven een score op het gebied van de afwezigheid, aanwezigheid, productiviteitsverlies en de afname van de activiteiten op het werk
