De OPHI II is een driedelig ergotherapeutisch evaluatie instrument om zowel de kwalitatieve als kwantitatieve gegevens over het handelen van de cliënt in kaart te brengen. Thema’s binnen het interview zijn: rollen, dagelijkse routine, handelingssituaties, activiteit/handelingskeuzes, beslissende levensgebeurtenissen. De therapeut verzamelt gegevens over de levensgeschiedenis en kan deze grafisch weergeven en scores op drie scoreschalen toekennen. Een overzicht van zwakke en sterke kanten van de cliënt op het gebied van handelingscompetentie, -identiteit en van omgevingsinvloeden kan een handvat bieden voor het opstellen van behandeldoelen.
