De Neck Pain and Disability Scale (NPDS) is ontwikkeld als een omvangrijk/multidimensionaal meetinstrument voor nekpijn. De vragenlijst bevat 20 items over de vier dimensies: intensiteit van de pijn, beperkingen in dagelijkse activiteiten, effect van nekpijn op de emotie en cognitie en problemen met de nek. Na elke vraag volgt een Visual Analogue Scale (VAS) met een range van 0 (normale functie) t/m 5 (meest beperkte functie). Hoe hoger een patient scoort op de NPDS des te meer beperkingen hij ervaart.
