Scale Overview
The Lambeth Disability Screening Questionnaire – LDSQ was developed by Donald L. Patrick, S. C. Darby, S. Green (1981). It is designed to measure Functional disability and physical limitation. The scale is intended for use with Adults.

Respondents rate each item using a dichotomous Yes/No format.
Scoring
Items are summed to produce a total score.
Total scores range from 0 to 22. Interpretation guidelines:
- No reported disability: 0 – 0
- Presence of functional limitations/disability: 1 – 22

Administration
