Scale Overview
The Lambeth Disability Screening Questionnaire – LDSQ was developed by Donald L. Patrick, S. C. Darby, S. Green (1981). It is designed to measure Physical and functional disability. The scale is intended for use with Adults.
Scale Structure
This instrument consists of 22 items organized into 3 factors/subscales: Activities of Daily Living and Mobility, Sensory and Communication, Instrumental Activities and Social/Work Role.

Response Format
Respondents rate each item using a dichotomous Yes/No format.
Scoring
Items are summed to produce a total score.
Total scores range from 0 to 22. Interpretation guidelines:
- No reported disability: 0 – 0
- Presence of disability (further assessment recommended): 1 – 22
Administration
The scale is self-administered and typically takes approximately 5 minutes to complete. It can be administered individually or in group settings. No special training is required for administration.
