Scale Overview
The Functional Status Questionnaire – FSQ was developed by Alan M. Jette, Priscilla D. Cleary, et al. (1986). It is designed to measure Functional Status. The scale is intended for use with Adults / Primary care patients.
Scale Structure
This instrument consists of 34 items organized into 7 factors/subscales: Basic ADL, Intermediate ADL, Mental Health, Work Performance, Social Activity, Quality of Interaction, Single Items.

Response Format
Respondents rate each item using a custom response format.
Response anchors: 4 = Usually did with no difficulty, 3 = Some difficulty, 2 = Much difficulty, 1 = Usually did not do because of health, 0 = Usually did not do for other reasons.
The scale includes 7 reverse-scored items to reduce acquiescence bias.
Scoring
Items are scored by subscale, with each subscale sum representing a distinct dimension.
Psychometric Properties
Internal Consistency: The scale has demonstrated good to excellent internal consistency with reported Cronbach’s alpha values of Basic ADL (0.77-0.84), IADL (0.82-0.89), mental health (0.77-0.88), social activity (0.65-0.83), quality of social interaction (0.42-0.79).
Administration
The scale is self-administered and typically takes approximately 10 minutes to complete. It can be administered individually or in group settings. No special training is required for administration.
