The Functional Assessment Inventory – Personal Capacities Questionnaire (FAI-PCQ) was developed by Nancy M. Crewe, Guy T. Athelstan (1984). It is designed to measure Functional capacity and vocational limitations. The scale is intended for use with Adults undergoing vocational rehabilitation.

This instrument consists of 30 items organized into 3 factors/subscales: Cognitive and Sensory Capacities, Physical and Motor Capacities, Vocational and Social Factors.

Scoring
Items are summed to produce a total score.
Total scores range from 0 to 90. Interpretation guidelines:
- Mild Functional Limitations: 0 – 30
- Moderate Functional Limitations: 31 – 60
- Severe Functional Limitations: 61 – 90
