Scale Overview
The EuroQol 5-Dimension 5-Level Quality of Life Scale – EQ-5D-5L was developed by EuroQol Group (2005). It is designed to measure Health-related quality of life. The scale is intended for use with General population and clinical groups.
Scale Structure
This instrument consists of 5 items organized into 5 factors/subscales: Mobility, Self-Care, Usual Activities, Pain/Discomfort, Anxiety/Depression.

Response Format
Respondents rate each item using a custom response format.
Response anchors: 1 = No problems / Not anxious or depressed, 2 = Slight problems / Slightly anxious or depressed, 3 = Moderate problems / Moderately anxious or depressed, 4 = Severe problems / Severely anxious or depressed, 5 = Unable to walk or perform activities / Extreme pain or depression.
Scoring
Items are scored by subscale, with each subscale sum representing a distinct dimension.
Total scores range from 5 to 25. Interpretation guidelines:
- No health problems reported: 5 – 5
- Presence of health problems (higher scores indicate greater severity): 6 – 25
Administration
The scale is self-administered and typically takes approximately 2 minutes to complete. It can be administered individually or in group settings. No special training is required for administration.
