Scale Overview
The Crush Diabetes Knowledge, Attitudes, and Beliefs Survey (CDKA) was developed by Pari Mokhtari, Kristine C. Jordan, Andrew Wilson, Brittany Sisco-Taylor, Julie M. Metos (2023). It is designed to measure Diabetes knowledge, attitudes, beliefs, and intent to change. The scale is intended for use with Adolescents (middle school students aged 12-14).
Scale Structure
This instrument consists of 25 items organized into 2 factors/subscales: Knowledge, Attitudes, Beliefs, and Intent to Change.

Response Format
Respondents rate each item using a custom response format.
Response anchors: True = True, False = False, DK = I do not know, 1 = Strongly Disagree, 2 = Disagree, 3 = Neutral/Undecided, 4 = Agree, 5 = Strongly Agree.
The scale includes 2 reverse-scored items to reduce acquiescence bias.
Scoring
Items are scored by subscale, with each subscale sum representing a distinct dimension.
Total scores range from 0 to 65. Interpretation guidelines:
- Knowledge Score Range (Correct answers scored as 1, incorrect/don't know as 0): 0 – 12
- Attitudes, Beliefs, and Intent to Change Score Range (Higher scores indicate more positive attitudes, beliefs, and intent): 13 – 65
Psychometric Properties
Internal Consistency: The scale has demonstrated good to excellent internal consistency with reported Cronbach’s alpha values of Cronbach's alpha: 0.70 (Knowledge), 0.70 (Attitudes, Beliefs, and Intent to Change).
Administration
The scale is self-administered and typically takes approximately 10 minutes to complete. It can be administered individually or in group settings. No special training is required for administration.
