Scale Overview
The Crush Diabetes Knowledge, Attitudes, and Beliefs Survey – CDKA was developed by Pari Mokhtari, Kristine C. Jordan, Andrew Wilson, Brittany Sisco-Taylor, Julie M. Metos (2023). It is designed to measure Diabetes knowledge, attitudes, beliefs, and behavioral intentions. The scale is intended for use with Adolescents (middle school students aged 12-14).
Scale Structure
This instrument consists of 25 items organized into 8 factors/subscales: Insulin, General Information, Prevention, Untrue Definition, Intent to Change, Attitudes and Beliefs: Lifestyle, Attitudes and Beliefs: Psychosocial, Attitudes and Beliefs: Emotional.

Response Format
Respondents rate each item using a custom response format.
Response anchors: True = True (Items 1-12), False = False (Items 1-12), DK = I do not know (Items 1-12), 1 = Strongly Disagree (Items 13-25), 2 = Disagree (Items 13-25), 3 = Neutral (Items 13-25), 4 = Agree (Items 13-25), 5 = Strongly Agree (Items 13-25).
The scale includes 2 reverse-scored items to reduce acquiescence bias.
Scoring
Items are scored by subscale, with each subscale sum representing a distinct dimension.
Total scores range from 0 to 77. Interpretation guidelines:
- Scored by subscale. Knowledge subscale ranges from 0 to 12 (correct answers). Attitudes, Beliefs, and Intent subscale ranges from 13 to 65.: 0 – 77
Psychometric Properties
Internal Consistency: The scale has demonstrated good to excellent internal consistency with reported Cronbach’s alpha values of Cronbach's alpha = 0.70 (Knowledge), 0.70 (Attitudes, Beliefs, and Intent).
Administration
The scale is self-administered and typically takes approximately 10 minutes to complete. It can be administered individually or in group settings. No special training is required for administration.
