Scale Overview
The Cohen-Hoberman Inventory of Physical Symptoms – CHIPS was developed by Sheldon Cohen, Harry M. Hoberman (1983). It is designed to measure Physical Symptoms Distress. The scale is intended for use with Adults.

Response Format
Respondents rate each item using a custom response format.
Response anchors: 0 = Not at all, 1 = A little, 2 = Moderately, 3 = Quite a bit, 4 = Extremely.
Scoring
Items are summed to produce a total score.
Total scores range from 0 to 132. Interpretation guidelines:
