Scale Overview
The Attitudes to Surgical Checklist Questionnaire (ASCQ) was developed by O’Connor, Reddin, O’Sullivan, O’Duffy, & Keogh (2013). It is designed to measure Attitudes towards surgical checklists. The scale is intended for use with Operating theatre personnel.
Scale Structure
This instrument consists of 24 items organized into 5 factors/subscales: Norms, Impact on teamwork & safety, Support, Initiate, Barriers.

Response Format
Respondents rate each item using a 5-point Likert scale (1 = Strongly Disagree to 5 = Strongly Agree).
The scale includes 1 reverse-scored items to reduce acquiescence bias.
Scoring
Items are scored by subscale, with each subscale sum representing a distinct dimension.
Total scores range from 24 to 120. Interpretation guidelines:
- Negative attitude towards surgical checklists: 24 – 56
- Neutral attitude towards surgical checklists: 57 – 88
- Positive attitude towards surgical checklists: 89 – 120
Psychometric Properties
Internal Consistency: The scale has demonstrated good to excellent internal consistency with reported Cronbach’s alpha values of Norms (0.70), Impact on teamwork and safety (0.84), Support (0.73), Initiate (0.87), Barriers (0.56).
Administration
The scale is self-administered and typically takes approximately 5 minutes to complete. It can be administered individually or in group settings. No special training is required for administration.
