Response Format
Respondents rate each item using a custom response format.
Response anchors: 1 = Level 1 (Most favorable/No difficulty), 2 = Level 2, 3 = Level 3, 4 = Level 4, 5 = Level 5, 6 = Level 6 (Least favorable/Severe difficulty).
The scale includes 11 reverse-scored items to reduce acquiescence bias.

Items are scored by subscale, with each subscale sum representing a distinct dimension.
Total scores range from 0 to 1. Interpretation guidelines:
- Utility score range (0.00 representing death equivalent, 1.00 representing full health): 0 – 1
Psychometric Properties
Internal Consistency: The scale has demonstrated good to excellent internal consistency with reported Cronbach’s alpha values of Alpha values for subscales range from 0.70 to 0.90.
Administration
The scale is self-administered and typically takes approximately 8 minutes to complete. It can be administered individually or in group settings. No special training is required for administration.
