The Anxiety Disorder Diagnostic Questionnaire – ADDQ was developed by Norton & Robinson (2010). It is designed to measure Clinical fear and anxiety. The scale is intended for use with Clinical and nonclinical populations.

This instrument consists of 8 items organized into 3 factors/subscales: Fear, Anxiety/Worry, Interference and Distress.

Response Format
Respondents rate each item using a custom response format.
Response anchors: 0 = None, 1 = 1, 2 = Mild, 3 = 3, 4 = Moderate, 5 = 5, 6 = Severe, 7 = 7, 8 = Very Severe.
Scoring
Items are scored by subscale, with each subscale sum representing a distinct dimension.
Total scores range from 0 to 32. Interpretation guidelines:
- Mild Severity / Interference: 0 – 8
- Moderate Severity / Interference: 9 – 16
- Severe Severity / Interference: 17 – 24
- Very Severe Severity / Interference: 25 – 32
