De CHART is een vragenlijst om de verschillende dimensies van handicap in kaart te brengen. De vragenlijst gaat over oriëntatie, fysieke mogelijkheden, mobiliteit, beroep, sociale integratie en zelfverzorging van de patiёnt. Zij bestond oorspronkelijk uit 27 items maar is daarna aangepast naar 32 items. Met behulp van dit instrument kunnen de moeilijkheden van de patiёnt in kaart worden gebracht, ook in de jaren na de rehabilitatie. Het maximale aantal te scoren punten is 500 en betekent geen handicap.
