The Organizational Practices Questionnaire (OPQ) was developed by Díaz Cabrera, D., Hernandez-Fernaud, E., & Diaz, R. I. (2007). It is designed to measure Organizational safety culture values and practices. The scale is intended for use with Maintenance personnel and organizational workers.
Scale Structure
This instrument consists of 48 items organized into 7 factors/subscales: Incident and Accident Reporting System, Safety Standard Procedures and Safety Rules, Safe Behaviour Promotion, Communication Systems, Working Style of the Immediate Superior, Job Satisfaction, Training Courses.

Response Format
Respondents rate each item using a custom response format.
Response anchors: 1 = Strongly Disagree / Very Dissatisfied / Never, 2 = Disagree / Dissatisfied / Rarely, 3 = Slightly Disagree / Slightly Dissatisfied / Seldom, 4 = Neither Agree nor Disagree / Neutral, 5 = Slightly Agree / Slightly Satisfied / Sometimes, 6 = Agree / Satisfied / Often, 7 = Strongly Agree / Very Satisfied / Always.
The scale includes 2 reverse-scored items to reduce acquiescence bias.
Scoring
Items are scored by subscale, with each subscale sum representing a distinct dimension.
Total scores range from 48 to 336. Interpretation guidelines:
- Low Safety Culture / Practice Alignment: 48 – 144
- Moderate Safety Culture / Practice Alignment: 145 – 240
- High Safety Culture / Practice Alignment: 241 – 336
Psychometric Properties
Internal Consistency: The scale has demonstrated good to excellent internal consistency with reported Cronbach’s alpha values of Adequate reliability (see Diaz-Cabrera et al., 2007).
Administration
The scale is self-administered and typically takes approximately 15 minutes to complete. It can be administered individually or in group settings. No special training is required for administration.
