Scale Overview
The Opioid Risk Tool – ORT was developed by Webster LR (2005). It is designed to measure Opioid abuse risk. The scale is intended for use with Adults prescribed opioids for chronic pain.
Scale Structure
This instrument consists of 10 items organized into 5 factors/subscales: Family History of Substance Abuse, Personal History of Substance Abuse, Age, History of Preadolescent Sexual Abuse, Psychological Diseases.

Response Format
Respondents rate each item using a dichotomous Yes/No format.
Scoring
Items are summed to produce a total score.
Total scores range from 0 to 26. Interpretation guidelines:
- Low Risk: 0 – 3
- Moderate Risk: 4 – 7
- High Risk: 8 – 26
Administration
The scale is self-administered and typically takes approximately 3 minutes to complete. It can be administered individually or in group settings. No special training is required for administration.
