Scale Overview
The Operating Theatre Human Factors Questionnaire – Attitude Scale (OTHFQ) was developed by O’Connor, Keogh, & Ryan (2012). It is designed to measure Human factors and teamwork attitudes in surgical environments. The scale is intended for use with Operating theatre personnel (surgeons, nurses, anesthetists).
Scale Structure
This instrument consists of 22 items organized into 4 factors/subscales: My Stress, Stress of Others, Communication, Command Responsibility.

Response Format
Respondents rate each item using a 5-point Likert scale (1 = Strongly Disagree to 5 = Strongly Agree).
The scale includes 7 reverse-scored items to reduce acquiescence bias.

Items are scored by subscale, with each subscale sum representing a distinct dimension.

Internal Consistency: The scale has demonstrated good to excellent internal consistency with reported Cronbach’s alpha values of Cronbach's alpha ranged from 0.41 to 0.66.
Administration
The scale is self-administered and typically takes approximately 10 minutes to complete. It can be administered individually or in group settings. No special training is required for administration.
