The EuroQol 5-Dimension 5-Level Quality of Life Scale – EQ-5D-5L was developed by EuroQol Group (2005). It is designed to measure Health-related quality of life. The scale is intended for use with General population and clinical cohorts.

This instrument consists of 5 items organized into 5 factors/subscales: Mobility, Self-Care, Usual Activities, Pain / Discomfort, Anxiety / Depression.

Response Format
Respondents rate each item using a custom response format.
Response anchors: 1 = No problems / No pain or anxiety, 2 = Slight problems / Slight pain or anxiety, 3 = Moderate problems / Moderate pain or anxiety, 4 = Severe problems / Severe pain or anxiety, 5 = Unable to do / Extreme pain or anxiety.
Scoring
Items are scored by subscale, with each subscale sum representing a distinct dimension.
Total scores range from 5 to 25. Interpretation guidelines:
- No health problems reported (11111): 5 – 5
- Presence of health problems (ranging from slight to extreme): 6 – 25
Psychometric Properties
Internal Consistency: The scale has demonstrated good to excellent internal consistency with reported Cronbach’s alpha values of High test-retest reliability.
Administration
The scale is self-administered and typically takes approximately 3 minutes to complete. It can be administered individually or in group settings. No special training is required for administration.
