Scale Overview
The Jefferson Scale of Physician Empathy – JSPE was developed by Mohammadreza Hojat, Salvatore Mangione, Thomas J. Nasca, et al. (2001). It is designed to measure Empathy in health professions. The scale is intended for use with Physicians, medical students, and health professionals.
Scale Structure
This instrument consists of 20 items organized into 3 factors/subscales: Perspective Taking, Compassionate Care, Standing in Patient’s Shoes.

Response Format
Respondents rate each item using a 7-point Likert scale (1 = Strongly Disagree to 7 = Strongly Agree).
The scale includes 10 reverse-scored items to reduce acquiescence bias.
Scoring
Items are summed to produce a total score.
Total scores range from 20 to 140. Interpretation guidelines:
- Low Empathy: 20 – 79
- Moderate Empathy: 80 – 114
- High Empathy: 115 – 140
Psychometric Properties
Internal Consistency: The scale has demonstrated good to excellent internal consistency with reported Cronbach’s alpha values of 0.80 – 0.85.
Administration
The scale is self-administered and typically takes approximately 10 minutes to complete. It can be administered individually or in group settings. No special training is required for administration.
