Scale Overview
The Stroke Impact Scale – SIS 3.0 was developed by Pamela W. Duncan, Robin K. Bode, S. Min Lai, Sue Min Perera (2003). It is designed to measure Stroke recovery and health-related quality of life. The scale is intended for use with Stroke survivors.
Scale Structure
This instrument consists of 60 items organized into 9 factors/subscales: Strength, Memory and Thinking, Emotion, Communication, Activities of Daily Living, Mobility, Hand Function, Participation, Stroke Recovery.

Response Format
Respondents rate each item using a custom response format.
Response anchors: 1 = Extremely difficult / Could not do at all / No strength / All of the time, 2 = Very difficult / A little strength / Most of the time, 3 = Somewhat difficult / Some strength / Some of the time, 4 = A little difficult / Quite a bit of strength / A little of the time, 5 = Not difficult at all / A lot of strength / None of the time.
The scale includes 3 reverse-scored items to reduce acquiescence bias.
Scoring
Items are scored by subscale, with each subscale sum representing a distinct dimension.
Total scores range from 0 to 100. Interpretation guidelines:
- Domain scores range from 0 (maximum limitation) to 100 (no limitation): 0 – 100
Psychometric Properties
Internal Consistency: The scale has demonstrated good to excellent internal consistency with reported Cronbach’s alpha values of Cronbach's alpha ranges from 0.83 to 0.90 across domains.
Administration
The scale is self-administered and typically takes approximately 15 minutes to complete. It can be administered individually or in group settings. No special training is required for administration.
