The Freezing of Gait Questionnaire (FOGQ) was developed by Giladi N, Shabtai H, Simon ES, Biran S, Tal J, Korczyn AD (1999). It is designed to measure Freezing of Gait. The scale is intended for use with Parkinson’s disease patients, older adults.

Response Format
Respondents rate each item using a custom response format.
Response anchors: 0 = Never / Not at all / 0 seconds, 1 = Very rarely / Mildly / 1-2 seconds, 2 = About once a week / Moderately / 3-10 seconds, 3 = About once a day / Severely / 11-30 seconds, 4 = Whenever walking / Extremely / More than 30 seconds.
Scoring
Items are summed to produce a total score.
Total scores range from 0 to 24. Interpretation guidelines:
- No Freezing of Gait: 0 – 0
- Mild to Moderate Freezing of Gait: 1 – 12
- Severe Freezing of Gait: 13 – 24
Psychometric Properties
Internal Consistency: The scale has demonstrated good to excellent internal consistency with reported Cronbach’s alpha values of Cronbach's alpha of 0.84.
Administration
The scale is self-administered and typically takes approximately 5 minutes to complete. It can be administered individually or in group settings. No special training is required for administration.
