RECORD KEEPING

Necessary in most forms of therapy, notes on subject are cataloged and filed for future use, including reference and training purposes. These notes can be requisitioned by courts through subpoena. Notes may be detailed or vague, but are considered standard operating procedure for most psychotherapy practices.
