CASE REPORT

n. a detailed report which documents a patient’s medical or psychological condition. It is a collection of data which includes all records relevant to the case. These include signs and symptoms, laboratory and diagnostic tests, diagnoses, treatment, medications, and recommendations for follow-up.

CASE REPORT: “One to be kept on file for future reference, a case report is a detailed and descriptive record on the mental or psychological health of an individual.”
